Do you have experience supporting individuals in connecting with their communities and accessing services that improve their quality of life?

The Bureau, a leading community wellbeing charity, is seeking a dedicated and compassionate Social Prescribing Link Worker to join our team, working within Glossop Primary Care Network. You’ll be helping people to live healthier, more fulfilled lives by linking them to non-clinical support services, community groups, and activities that meet their individual needs.

About the Role:

As a Social Prescribing Link Worker, you will provide one-to-one personalised support to people who are referred by healthcare professionals or self-refer into the service. This role is about listening, understanding “what matters” to each person, and working together to co-create a support plan that helps them connect with services, reduce social isolation, and build resilience.

You’ll be based within GP practices in Glossop, working closely with healthcare teams, as well as community organisations, to provide holistic support to individuals. You will also have a key role in developing strong relationships with local groups and services, helping to shape a vibrant network of community support.
