Do you have a passion for helping people feel seen, heard, and supported? Are you organised, compassionate, and great at getting the right help to the right place at the right time?

We’re looking for a Triage Coordinator to join our social prescribing team — someone who can confidently handle referrals, offer a warm welcome, and help people take the first steps toward support.

In this role, you’ll be the vital link between health professionals, community services, and the people we support — making sure every referral is thoughtfully assessed and directed to the best place, whether that’s a Link Worker, a local group, or other services.

🧭 What you’ll be doing:

• Acting as the first point of contact for new referrals into the social prescribing service.

• Having supportive conversations (usually by phone) to understand people’s needs, priorities, and preferences.

• Matching people with the right Link Worker, community resource, or support pathway.

• Managing referral systems, databases, and case notes with care and attention to detail.

• Building strong working relationships with GP practices, community organisations, and other local partners.

• Someone with a warm, friendly manner and excellent communication skills.

• Confident using phone, email, and systems to keep things running smoothly.

• Great organisational skills — able to juggle tasks and work efficiently.

• A compassionate listener who can ask the right questions and respond with empathy.

• Experience in health, care, community work or admin is helpful — but most of all, we’re looking for the right person with a people-first approach.
