The Community Navigation service helps people who are struggling to manage, or live independently with a long term health condition.
We help people access a wide range of support services and activities provided by community groups and voluntary services across Glossopdale. Community Navigation is part of a wider initiative with local health services, Adult Social Care Services, police, fire services and voluntary agencies. Community Navigation is also known as Social Prescribing.
The service is available:
- 24 hours a day via our website
- Weekdays from the Bureau offices at Bank House
- Weekdays via home visiting
- Weekly through our outreach sessions run from Lambgates and Manor House GP surgeries, and at Shire Hill Hospital.
We offer a casework approach to work with the individual to address the most urgent presenting problem/s or issue, and then offer a follow up conversation about their wider situation to identify and help address the factors causing the problems. Where requested, the Community Navigator will provide periodic updates and details health improvement outcomes as part of a Care Plan. Before a case is closed we also provide a 3 month follow up to ensure the support services and activities put in place are achieving the desired outcomes.
All our staff have a recognised IAG qualification and have a successful track record of finding solutions for people experiencing complex or chaotic lifestyles, social isolation and long term health conditions.
